A reputable homecare agency in Manhattan is looking to hire a Registered Nurse (RN) with experience in homecare to provide exceptional care to patients in their homes. This full-time role offers a competitive salary ranging from $140k-$150k. The ideal candidate will have strong clinical skills, be compassionate, and possess excellent communication skills. This role involves providing comprehensive nursing care, coordinating patient care plans, and ensuring compliance with healthcare standards. This is a full time position (depending upon experience $140K-$150k base salary for full time work plus full benefits (401K, Healthcare, vacation)
Responsibilities
● Ensure the ongoing satisfaction, quality, and safety of existing clients through regular
contact (Manhattan NYC only), in conjunction with designated quality assurance home
visits (22 visit minimum per week)
● The position requires the exercise of discretion and independent judgment regarding
significant matters of quality assurances/clinical management/risk management and
training of the Caregivers

● Clear understanding of RN regulations and responsibilities for New York State as related
to nursing delegation to an unlicensed caregiver.

● Reports to Patient Care Manager and Owner as contact for questions pertaining to your
primary responsibilities.
● Reports to Patient Care Coordinator for daily tasks, schedule and responsibilities.
Primary Responsibilities:

● Perform Initial Nursing Assessments.

● Prepare and maintain a current Plan of Care outlining the specific specialized services
to be provided to the client. The Plan of Care must be updated at minimum every 6
months but whenever necessary to reflect a change in the client’s condition.
Documentation should be retained electronically and within the client’s file.
● Make sure there is a proper medication list for the client, pre-pour medications for clients
who have requested this service, and manage medication administration.
● Review, create and keep updated the medication administration and specialized services
records for clients receiving those services.
● Conduct quality assurance home visits with clients at minimum every 3-4 months to
supervise the overall quality of the service.
● Conduct quality assurance visits as needed to sick clients and clients receiving
medication administration or delegated specialized services.
● Conduct supervisory visits every 3-4 months to CAREGivers in the client’s home to
ensure proper delivery of the service.
● Collect service records for office files and keep client journals stocked with appropriate
documentation forms.
● Provide oversight, up to and including the instruction of initial and ongoing in-office and
field training for CAREGivers.
● Conduct ongoing in-office and field training for CAREGivers.
● Conduct specialized services training with individual CAREGivers who are serving high
needs clients receiving specialized services. The training must include a return
demonstration performed by the CAREGiver and signed off/dated by the nurse. This
documentation must be retained in the CAREGiver’s employment file.
● Keep detailed documentation of all relevant phone and in-person contact with clients,
client representatives, CAREGivers and others.
● Make staffing recommendations as required by the HR Staffing Coordinator.
● Communicate and report to the office any incidents and accidents observed during visits.
● Prepare Client Discharge Paperwork within the time required by the New York State
Department of Health.
● Act as an expert adviser regarding the medication services and delegated specialized
services training given to CAREGiver.
● Gain mastery of the following technical systems/processes/resources: (WellSky (formerly
ClearCare), Google Drive, Google Spreadsheets, Thinkific, EmailMeForm, WhatsApp,
phone messaging and more).
● Attend the Quality Improvement meetings quarterly.
● Maintain a current, non-restricted Registered Nursing License in the State of New York.

● Adhere to the requirements of the operations Handbook, as prepared by Home Care
Associates, Inc. dba as Home Instead Senior Care.
● Assist with the medication and other comprehensive clinical services as outlined in the
New York State Administrative Rules.
● Comfortably supervise compliance of Caregivers with the requirements of the Home
Instead Senior Care Handbook.
● Comfortably delegate nursing tasks as outlined by the New York State Department of
Health.
● Clear understanding of the rules and regulations regarding delegating nursing tasks to
both other licensed and non-licensed employees.
Qualification:
● Minimum 7 to 10 years of nursing experience, including delegation and management of
employees.
● Be willing to commute using the New York City public transportation visit to perform a
minimum of 4 client visits per day (22 per week).
