We are looking for a Care Manager experienced in CCMP home health care to join our dedicated team, providing quality services to clients in the Bronx area. The ideal candidate will have a deep understanding of CCMP processes, home health care regulations, and a compassionate approach to patient care. Responsibilities:

• Oversee and coordinate comprehensive care plans for patients in CCMP home health care.
• Ensure that all care plans meet the regulatory requirements and are tailored to individual patient needs.
• Collaborate with healthcare providers, caregivers, and family members to deliver high-quality patient care.
• Monitor and evaluate patient progress, making adjustments to care plans as needed.
• Serve as the primary point of contact for patients, families, and healthcare teams.
• Maintain accurate patient records and documentation in accordance with CCMP standards.

Qualifications:

• Proven experience in CCMP home health care.
• Strong understanding of home health care regulations and compliance.
• Excellent communication and interpersonal skills.
• Ability to manage multiple clients and tasks effectively.
• Compassionate and patient-focused approach to care.
• Relevant certifications in home health care or care management preferred.

Location: Bronx, NY

Salary: $65k-$100k, depending on experience.
